This request is to request a visit for reading from a reader from HS Veitur.

1. Type in sender, e-mail and phone
2. Type in requested reading date by clicking button at "Prefered reading date"
3. Type in ID number and select house by clicking button at House ID field
4. Type in notes or info in the field "Notes"
5. Click Test and then Send
