This service request is to return request that no special request template is
for. It is possible to send this request to several departments in the company.
1. Type in sender name, email and phone number.
2. Select department that the request should be delivered to.
3. Type in ID number Product payer and select house by clicking button on Houe
ID field
4. Fill inn field for description of the service requested, click Test and then
Send
